Senior Health Care at Your PACE
Orange County PACE is a comprehensive healthcare and wellness program designed to help seniors live independently at home as an alternative to nursing care.
Our network of dedicated healthcare professionals provides individualized, coordinated care from medical services to home medication delivery, transportation to nutritious meals – all services are personalized to meet your specific needs to live a happy, healthy life at home. Access to care is available 24 hours a day, 365 days a year.
The PACE team approach to care utilizes a unique, world-class delivery system designed to eliminate disparities in healthcare access for underserved populations, leading to successful outcomes for our diverse community of seniors and their families.

Orange County PACE is home to our primary care clinic, rehabilitation center, transportation hub, and a full interdisciplinary care team on site.
Enrolled members are invited to join us at the PACE Day Center for daily recreational activities and socialization with other members of our PACE community.
OUR SERVICES
All of Your Health Care. All in One Place.
As an Orange County PACE participant, you will receive primary care services from the physician at our health center. If you need care from a specialist, you may choose from one of the specialists within our provider network. Your dedicated care team and physician work closely with your network specialists, so that your care is carefully coordinated and designed to meet your unique needs.
The PACE Center
The PACE Center is where all the fun happens! Here, our participants get to socialize with other senior members of the Orange County PACE community while enjoying arts and crafts, playing games, watching movies, dancing to live musical performances, and so much more. And yes, there’s bingo.
Delicious breakfast and lunch meals are served under the guidance of a dietitian and light exercise sessions are offered to keep our seniors feeling their best.
ELIGIBILITY
You may be eligible to receive PACE services, with no out-of-pocket costs, if you:

- Are 55 years or older
- Live in the designated service area
- Are able to live safely in the community
- Meet the level of care requirement as certified by the California Department of Health Care Services long-term care division
OUR PROVIDERS
Interdisciplinary Team (IDT)
Interdisciplinary Team (IDT)
IDT is what makes health care at PACE so unique. The IDT provides and plans all care for our PACE members and works together to deliver care that is coordinated and specific to each individual member. Using a team approach to care, we partner with our participants to ensure their medical, social, and emotional needs are met.
Your IDT will consist of:
- Primary Care Physician
- Nurses
- Social Workers
- Activities Director
- Registered Dietician
- Medical specialists by appointment:
(Dentist, Audiologist, Podiatrist, Optometrist, etc.) - Personal Care Attendant or Preferred Caregiver
- Transportation Staff
- Occupational, Physical, and Speech Therapy
- Transportation
Once a care plan is in place, Orange County PACE will schedule all appointments and arrange transportation on the senior’s behalf.
Provider Network
Provider Network
As an Orange County PACE Participant, all of your Primary Care services are provided by your dedicated physician at our health center.
If you need a specialist or hospital care, you may choose from trusted, local providers within our Provider Network. All services provided in-network are at no cost, with no co-pays for the PACE member.
Our network of specialists work with your PACE Interdisciplinary Team to ensure you receive the level of care you need.

PACE is funded by Medicare and Medicaid to deliver necessary care.
Seniors who have Medi-Cal and/or Medicaid are eligible to receive PACE services with no out-of-pocket cost. Private pay options are available for seniors who are not covered by Medi-Cal and/or Medicaid Talk to a PACE representative to check your eligibility.
Upon enrollment in the program, the participant agrees to receive all health care services through PACE’s network of providers. All services and providers within the PACE network are available to the participant at no cost, with no co-pays. Participants may be liable for the cost of medical services from an out-of-network provider or without receiving prior authorization for service from PACE, except for emergency services.
